1. what operating system are you running? ( win2000? win XP? Vista? 7? 8? Win10?)
2. what browser do you use?
3. briefly describe the problem.
4. how long has it been happening?
5. what are you doing when the problem occurs?
6. any error messages accompanying the problem? (copying the exact message is great!)
7. what have you tried so far to remedy the problem?
