here is hans rosling on worldwide prostate cancer statistics and, among other things, the correlation between rising income and overdiagnosis:

gapminder.org

he is one of the world's leading thinkers. for those who may be unfamiliar with his fascinating TED talks and his visualization software, here is his bio:

his site has similar videos on other cancer types:

Very interesting.

Clearly the decision to treat or not must be on a case by case as I think Rosling suggests. I had a PSA that went from under 2 to 6 in a year. On biopsy, Gleason score was a 7. I saw 4 specialists ( 1standard radiation, 1 HIFU, 1 Davinci and the Duke stereotactic study). Not one of the specialists recommended watchful waiting.

I decided to go with the Duke program, and am very glad I did.

They get paid not to wait, Tom.

You're a trifle cynical, methinks.

The nice thing about a Gleason 7 in the context of watchful waiting is that you may not have long to wait.

Saves time.

Good thing you didn't go with them Death Panel folks:

This was an interesting (if strangely designed) study:

As I said before, I think MDV3100 prior to surgery makes a lot of sense.

For death panel politics, go check out an editorial about the Lockerbie bomber. Apparently, they let him out of jail because he only had about a month to live. When he got to Libya, the treatment was so much better that he lived an additional three years. Almost nothing was covered under the Scottish system. I cannot remember where I read this editorial, but it made for interesting reading.
