One thing to add... At one time the U.S. was arguably the best place in the world for tertiary medical care.

Not true anymore. Thailand, India, other destinations can provide excellent state-of-the-art services for a fraction of the cash -- however the consumer/patient had better do their research as there are places that will cut your hair, massage your, ummm, body, AND do a tooth implant or cancer treatment all in one facility, and it might be the same person doing it.
