Simply because Ryan proposes to give Seniors $1200 per year to buy insurance provided by private companies. This is nearly $200 less than the monthly premium for Part B. I had assumed, until I started writing this response, that Medicare Part A will continue as is. It may not but I will assume here that it continues along with the pay roll tax that finances it. Medicare Part B will be gone as present subscribers pass on. The result will be the end of Medicare Part B and its replaced with private insurance. Private insurance companies have, as I explained, substantially more overhead than Medicare. Therefore, while many will not be able to afford private insurance, the money spent by subscribers on Doctors, etc. will not be reduced. It may be increased a little. But the benefits paid will be reduced because the overhead (percentage of premiums that is not paid out in benefits) will be increased dramatically.

I hope this helps clear up the issue except for the Part A/Part B question. I suspect my assuption about Part A continuing is incorrect because it is Part A that is paid out of the payroll tax and, for which, there is expected to be a serious short fall. the Ryan proposal may be much worse than I thought.
