Boston and San Francisco (where the expenses were rising slowly) are definitely more urban - and probably have more MDs per capita - that Eastern Long Island (where they've been galloping).

I wonder if anyone ever tried to overlay the maps for percentage of patients over 85 or 90 years old dying in the hospital (vs. at home or hospice) and overall medical expenses per capita...
