Why wouldn't carriers pay for a signal that is expensive for local affiliates to make knowing cable companies and DTV make a profit from carrying that signal? I dumped CATV for DirecTV because the cable company would insert local paid commercials instead of public service or programming ads from the network. Right now, the fastest way to deploy MediaFlo nationwide would be to rent a transponder on a geosynchronous sat, something they already do for Omnitracks. Fiber is an option. But, birds offer universal coverage at a much lower risk of outages.
