Don: you are obviously highly knowledgeable and idealistic about libertarian economics. Well enough .Your example of Liberia however is telling. Millions of Africans have been condemned to die from HIV, in part because of the very reluctance to sell advanced ARV drugs at prices that would allow their use. In recent years, considerable pressure has been brought to bear on them to do so, by organizations like MSF and PIH, with some success. In your calculations, perhaps those deaths are a benefit.
