I just read an article shortly ago in our local paper and asked a guy I golf with if it is this easy and he assured me it is.

If someone begins to have balance trouble or seem off and you suspect a stroke,

1: Ask them to smile
2: Ask them to raise both arms to shoulder height
3: Ask them to speak a simple sentence

If they have trouble with any part of these call 9-11 immediately.

According to him you will see the smile as lopsided, they will not be able to raise both arms equally or they will be confused trying to speak a sentence. They can show these symptoms before they even realize they are having trouble.
